Instructions for Submitting Your Family Groupsheet

The form below is made for a family of 12 children. The chart will be filed under the FATHER's surname. Feel free to also submit a chart for the MOTHER's family. If there is more than one marriage, submit a second family groupsheet and make a notation in the husband's field that states this is an additional marriage. Use your Tab key to jump between fields.

Put all SURNAMES in capital letters!
DO NOT include LIVING PERSONS on these charts.
Add any additional information in the area left for notes, below the children.
